How to Verify the Fix and Codify the Diagnosis for Reuse
Once a fix has launched, confirm it actually worked and leave a case others can reuse. Set the target before acting, re-measure the same gap, rule out other causes, and file the closed case in a shared record.

Before the intervention starts, write what change will count as success and by when. 'Late tickets fall from six to under two within three weeks' is a target; 'should improve' is not. Log it at the moment you choose the fix, so a later result cannot be rationalized as progress.

Once the fix has had time to land, measure the same gap again with the original definition and record the number. Skipping the re-measure leaves you guessing whether anything changed. Compare it against the target you set beforehand.

Read the re-measure against the target and choose a clear next step: sustain, adjust, or escalate to a different cause. Treat a partial result as a signal to adjust, not a reason to call the case closed. A met target sustains the fix; no movement escalates.

When a gap improves, ask what else shifted during the same period before crediting the fix. Rule out the obvious alternative explanations so a change something else caused is not booked to your intervention. Only then credit the fix.

When the case closes, file it in a shared record with four fields: the original gap, the controlling cause, the fix, and the target versus the re-measured number. A colleague handling something similar should be able to follow the case from the record alone, without the original context.
Common Pitfalls
Avoid the common failure modes.- A fix gets chosen with no target for what success looks like. Write the target, with a date, at the moment you choose the fix.
- The case gets closed on impression instead of a re-measured number. Measure the same gap again with the original definition before deciding anything.
- The fix gets credited for a change something else caused. Ask what else shifted in the same period before crediting it.
