Feedback you can act on starts with the request, not the response. A general 'how am I doing' invites reassurance, while a request aimed at one piece of work and one thing you want to improve comes back specific enough to change something. Timing matters too: ask while you both still remember the detail, and when a first answer stays polite, ask the follow-up that gets the candid version.
